The study uses a variety of information gathering instruments, including a
scripted telephone interview and several printed questionnaires. Click

Parent Interview
The parent interview is conducted over the telephone in a process called a
Computer Assisted Telephone Interview (CATI). CATI interviews are scripted,
and this is the script. It includes questions about
the child's history, health, behavior, and education services.

Child Assessment
In Waves 1 through 5, nearly 300 local assessors were employed to administer a one-on-one assessment of children participating in PEELS. In addition, teachers and other service providers were asked to complete rating scales as part of the teacher questionnaires. Assessments varied depending on the age of the child.

Questionnaires
There are seven questionnaires. Together they describe the educational
environment for a child in the study from the state down to the individual teacher.
State Questionnaire
The state questionnaire is sent once to each State Education Agency (SEA)
"619" Coordinator or Adminstrator.
SEA Questionnaire
District Questionnaire
The district (or Local Education Agency) questionnaire is sent once to each
participating school district.
LEA Questionnaire
School/Program Questionnaires
Each school or early childhood program with a child in the study receives
a questionnaire. If the child is in an early childhood program, the program
director gets a questionnaire tailored to early childhood programs. If
the child attends a school, the principal receives a questionnaire tailored
to schools.

There are three teacher questionnaires; one for early childhood program teachers,
one for kindergarten teachers, and one for elementary school teachers.
